The primary goal of our Immunization program is to prevent the spread of vaccine-preventable diseases in Bay County. We offer free immunizations for uninsured and underinsured children ages 1 month - 18 yrs. Underinsured includes those in which insurance does not pay for immunizations. We also offer adult immunizations for a fee.

Clinic Hours
All immunization services are by appointment only.
Please call (850) 872-4455 and press 1 for Appointments then press 1 for Medical Appointments to schedule an immunizations appointment for the following services:
- Child Vaccines
- Adult Vaccines
- Out of state records transferred to Florida 680 Form (for school registration)
- 680 Form
- College Forms (for college entry)
- Religious Exemption
- Immigration Vaccines
- International Travel Vaccines
For Questions about vaccine requirements for international travel please visit Travelers' Health | CDC and select your destination.
For record transfers, 680 Form and College Forms, please bring the following:
- Copy of Immunization records (must be translated to english)
- Parent picture ID/Passport
- If needed, a copy of guardianship paperwork (from DCF, Court Docs, or Power of Attorney)
- Insurance card (if applicable)
- Optional: Birth certificate and SSN card
